Children Present At The
Appointment Time
Due to OSHA regulations, and for the
safety of your children, no child will be allowed in the back office.
Please make necessary arrangements for your appointment. We are enforcing
this policy effective immediately. No child under 12 years of age will
be allowed to wait in the waiting room alone. Again, this is for the
safety of your children.
Missed Appointments
Our goal is to provide quality care to all
of our patients. When an appointment is scheduled, a block of time in
the doctor's schedule has been allotted to you, the patient, to allow
adequate time to address all of your concerns and provide medical
treatment.
We reserve the right to charge a $25.00
fee for all missed appointments and appointments that have not been
cancelled 24 hours prior to the scheduled appointment time.
Late Appointments
All patients
that arrive more than 15 minutes late for a scheduled appointment will
be rescheduled. This does not apply if prior arrangements have been
made.

Most insurances are accepted. The
